What is it?
Access Services Community Based Care Management program works with individuals experiencing acute mental illness to secure stable permanent housing.  Working in partnership with other community stakeholders, this pilot program assists individuals with unsecure housing transition from a hospital stay to independent living in the community. Our team works directly with individuals to identify housing that meets their needs and assists them with securing these units.  Â
Additionally, our team works with clients to strengthen their connection to other community resources. Once housed, clients are provided ongoing housing stability support and assistance with developing and maintaining a plan toward being able to maintain their housing independently.  
Housing Stability Specialist
Our Housing Stability Specialist focuses on helping individuals to stabilize housing and connect to public benefits, employment support, and additional services. The Housing Stability Specialist also serves as a community support to individuals in maintaining their housing. If the person desires additional support, we provide referrals to our Starting Point team and other collaborative community partners.Â
Housing Locator
Our Housing Locator establishes and maintains landlord relationships. They also negotiate and facilitate lease expectations and move-in requirements. They engage in collaborative relationships with housing insecure individuals, mental health providers and housing providers to secure stable housing support.
